Our SmileLABS™ Customer Purchase Agreement Pads, which incorporates the liability waiver and hold harmless agreement, are printed carbonless 2-part NCR paper. We recommend that all customers read, fill out, sign and date one of these forms prior to performing the SmileLABS™ Cosmetic Teeth Whitening Treatment on themselves. These forms are also countersigned and dated by the SmileLABS™ Cosmetic Teeth Whitening Authorized Dealer, and a copy is given to the customer after the treatment is completed. There are places on the form where the price paid and the before and after shade number from the shade guide can be written, so this also serves as a receipt and treatment results record.
